The star starfish drawing is modeled after a childish drawing of an anus, from the 1973 book Breakfast of Champions by Kurt Vonnegut, who is mentioned by name in the description of this item.
According to a UK Swearing Dictionary, Roger's Profanisaurus, the chocolate starfish is a noun meaning "Rusty sheriff's badge (qv); Bovril bullet hole."
Chocolate starfish may also be a reference to the Limp Bizkit album of 2000, Chocolate Starfish and the Hot Dog Flavored Water.
